Randolph-Asheboro YMCA Liability Waiver and Disclosure
Our school, ______________________________, agrees to participate in the 11th Annual Bank of the Carolinas
JV Holiday Classic Basketball Tournament. All games will be played at the Randolph-Asheboro YMCA, Southwestern
Randolph High School, Randleman High School, and Southwestern Randolph Middle School. By entering the
tournament, our school realizes that no insurance will be provided to players, coaches, teachers, staff, or volunteers.
We also agree to hold harmless the Randolph County Board of Education and the Randolph-Asheboro YMCA, its
Board of Directors, staff members, or sponsors for any theft, injury, sickness, or death as a result of participation in
the tournament which will be held on December 26, 28, & 29, 2009.
Even though there is no entry fee being charged for this tournament, it is the responsibility of the principal and
athletic director to make sure your teams participate in all three days of the tournament. Once this form is signed, it is
considered a firm commitment from your school to play in our tournament because this form will be used to draw up
the tournament brackets. Because it creates havoc with the brackets if a team does not show up for all of its games,
your school will be charged $150 per team per day in the event they do not show up for their designated games at
their designated gym, and at their designated times.
To officially enter your team, your school needs to have all appropriate personnel sign the form IN ITS ENTIRETY
and return it to David Cross by no later than November 23, 2009. Failure to return the form may result in your school
being excluded from the tournament. Please complete ONLY the sections of the teams that you will be sending.
